BCHN Honors Health Care Heroes at Recognition Dinner

BCHN honored patients, community partners and health leaders at the 13th Annual Community Health Champions Recognition Dinner on September 6.   Hosted by BronxNet's Rhina Valentin, the evening featured patients sharing their inspiring success stories working with community health workers to deal with issues such as housing and job insecurity, new motherhood and pre-diabetes.

Held annually during National Health Center Week, this year's theme was "Health Care Heroes".  

CHCANYS director of corporate relations and meeting support Roxanne Wynn-Trotman presented BCHN chief executive officer Eleanor Larrier and Board of Directors president Zahra Elmekkawy with a proclamation from New York City Mayor Bill de Blasio.

Renee Patterson Shares Story of Seton Falls Park Turnaround

When BCHN's Bronx REACH Champs program formed a parks and open spaces coalition in the spring of 2016, Seton Falls Park Preservation Coalition president Renee Patterson was one of the first members to join.  The goal of the team was to increase use and physical activity in seven Bronx parks, including Seton Falls.  Renee was having trouble getting people to come to the beautiful 30-acre nature park she represented in the Northeast Bronx.   Two years later, the park is active, including a "Fitness in the Park" class you will see in the video above.

BCHN Presents SDH Poster at NACHC Conference in Orlando
BCHN sent a team of Board members and staff to this year's National Association of Community Health Centers (NACHC) Community Health Institute (CHI) in Orlando. 

director of programs Tashi Chodon and quality improvement coordinator Winsome Ramsey presented a poster which 
highlighted BCHN's work "Addressing Social Determinants of Health Through Integrating Community Health Workers into Health Center PCMH Care Teams." It was developed by Chodon, community health programs developer Renee Whiskey, and grants and program developer Damiris Perez Agu. Click here to view the poster.

The annual national conference draws thousands of health center clinicians, nurses, CEOs, board members and advocates from around the U.S. each year. 

New data shows that America's Health Centers serve more patients than ever at 28 million in 11,000 communities across the country. 
West Farms Family Practice Group Walks to Zoo on Thursdays

An enthusiastic walking group from West Farms Family Practice makes their way to the Bronx Zoo and back every Thursday morning.  The group is led by BCHN community health worker Pedro Gonzalez.
